CONSENT FORM/T&C's OF ENTRY

I give my permission for my child/ren to attend the school MOVIE NIGHT SAT 24 FEB and for first aid to be administered should it be needed. I understand the emergency details listed above will be used to contact me if required unless otherwise advised. I also agree to the T&C's below and understand that if this consent form is not signed, admission to the movie night will not be granted.

• The £6 per 1 x ticket entry fee includes a light snack, drink and donation to the PTA.


• Tickets are limited to 80 spaces, are for St Peter's students only, and will be issued on a 'first come, first served' basis.


• Tickets are non-refundable. However, tickets may be transferred to another student of St. Peters Primary School provided the PTA are notified in advance.


• Entry is subject to receipt of this signed/submitted consent/T&C's form. Without the consent form, we are unable to allow entry.


• Doors will be open from 2.40pm and the movie will begin at 3.00pm. Please arrive promptly.


• The movie is approx 90mins and due to finish at 4.30pm. Please arrive on time to collect your child/children.


• There will be NO INTERMISSION. 


• Students will be seated on the floor and so are encouraged to bring their own blanket, bean bag, and/or pillow.


• Students must remain seated during the movie. If a student is unable to do so or becomes disruptive, a parent or guardian will be required to collect the student promptly.
